Course Objectives
In addition to a thorough knowledge of the patients that you will evaluate the following is a list of educational objectives of the third year ambulatory rotation in pediatrics: I. Normal Development/Growth A. Know one milestone of the gross motor, fine motor, language, and personal/social categories for ages: 2 weeks, 3 months, 6 months, 9 months, 1 year, 18 months, 2 years, 3 years, 4 years, 5 years, and 6 years. B. Be familiar with the Denver Developmental flowchart. C. Be familiar with the work-up of the failure to thrive, tall and short stature, obesity, precocious and delayed puberty. II. Behavior A. Be familiar with normal eating patterns from infancy to age 1 (breast, bottle, and solids). B. Be familiar with dealing with the crying healthy child (colic). C. Know a few basics on the prevention of sleep problems (night awakenings, bedtime resistance, nightmares, night terror, sleepwalking) D. Know a few basics on disciplining young children (time-out techniques, terrible twos, temper tantrums) E. Know some basics on thumb sucking, breath holding spells, pacifier misuse, toilet training, stuttering vs. nl. fluency, attention deficit disorder, enuresis, encopresis, excessive TV and video use. III. Skin A. Know care of newborn skin (bathing, diaper rash, circumcision). B. Most rashes are caused by antibiotics, chicken pox, heat rash, measles vaccine, birthmarks, benign newborn rashes, roseola, scarlet fever, sunburn, and hives. Be familiar with their identity and treatment. C. Know the management of eczema, acne, common fungal infections, contact dermatitis, impetigo, scabies, lice, and warts. D. Know the most common causes of hair loss. IV. Eyes A. Know the three causes of amblopia: strabismus, focusing problems, and cataracts. B. Know how to identify and treat eye allergies, pinkeye, corneal abrasions and styes, and blocked tear ducts V. Ears A. Be comfortable in diagnosing and treating otitis media, external otitis, packed earwax, and swimmerâs ear. VI. Nose/Lungs A. Know outpatient treatment of viral and allergic nasal symptoms, sinus problems, and cough. B. Know basic nosebleed management. C. Know the differential diagnosis of wheezing in infancy. D. Know the management of asthma, croup, stridor, and pneumonia. VII. Mouth/Throat A. Know how to treat canker sores, cold sores, sore throat, toothache, and teething. B. Know the risks and benefits of tonsillectomy & adenoidectomy. VIII. Heart/Hematology A. Know the names of the benign heart murmurs. B. Know the most common types of congenital heart disease (acyanotic). C. Know the most common types of cyanotic heart lesions. D. Know how to place pediatric leads for an EKG. E. Know the work-up and differential for hypertension F. Know the work-up for anemia and bleeding disorders. IX. Abdomen A. Know an expanded differential and work-up for the acute abdomen (i.e. appendicitis, intussusception, torsed ovary). B. Know how to manage constipation. C. Know how to manage acute diarrhea and vomiting and know the work-up for chronic diarrhea. D. Know how to assess dehydration and the basics of rehydration. E. Know treatment for excessive gas, hiccups, nausea, pinworms, anal fissure. F. Know the work-up and differential for upper and lower gastrointestinal bleeding. X. Genitals/Urology A. Be able to identify ambiguous genitalia. B. Know the work-up and differential for scrotal and groin swelling. C. Know how to treat UTIs and the follow-up for these patients. D. Know the basics on foreskin retraction and its problems and the causes and treatment of prepubertal vaginitis. E. Know the work-up for undescended testicles. F. Know the work-up and differential for hematuria. IX. Bones/Joints A. Know a differential and work-up for back, hip, knee, and ankle pain and swelling. B. Know the work-up and differential for limp. C. Know the most common congenital malformations of the upper and lower extremities (congenital hip, metatarsus adductus, femoral anteversion, tibial torsion, pes planus, pes cavus, bowlegs). D. Know how to screen for scoliosis E. Know how to treat sprains and strains XII. Immunizations A. Know the current schedule of immunizations. B. Know the side-effects and nature of MMR, Polio, Hib, Hepatitis, and DPT. C. Know how to administer the above. XIII. Preventive Pediatric Health Care A. Be familiar with screening measurements, labs, etc., as outlined on the next page. |
