Bio-Med 450: The Pediatric Clerkship

Diagnosis: fever, R/o serious bacterial infection

Condition: guarded

Vital Signs q 4h

Allergies: NKDA

Activity: as tolerated for age

Diet: Breastfeeding po ad lib

IVFs: D5 1/4 NAA at 15 ml/hr for maintenance or 5 ml/hr to keep vein open

Medications:

  • Ampicillin 190 mg IV q 6h
  • Gentamicin 15 mg IV q 12h
  • Acetaminophen (80 mg/0.8 ml) 40 - 60 mg (0.4 to 0.6 ml or 1/2 to 3/4 dropperful) po q 4 -6 h prn T > 100.5

Labs:

  • CBC with diff, blood culture
  • Urinalysis and urine culture
  • Cerebrospinal fluid
    • Tube #1 gram stain and culture
    • Tube #2 glucose and protein
    • Tube #3 cell count and diff
    • Tube #4 consider HSV PCR or Enterovirus PCR
