To: Brown University Investigators

Complete an Application for IACUC review for all research, teaching or testing projects involving animals.  The primary form of the Application is the “Application for the Use of Vertebrate Animals in Research or Teaching,” which must be submitted for ALL animal use.  In addition, most projects will require the completion of 1 or more Appendixes. 

Sign and return the completed Application, including relevant Appendixes, to the IACUC Coordinator in the Research Protections Office. Box 1986, Brown Office Building.  

Address administrative questions regarding the Application to the IACUC Coordinator.   The campus veterinarians are available for consultation on animal care or animal modeling issues.   Note: veterinary consultation is required for all projects that could potentially produce pain or distress.

Appendices:

Appendix 1-Antibody Production: Complete if the study design includes the production of polyclonal or monoclonal antibodies in animals.  This also includes collaborative or contract procedures at external facilities.

Appendix 2-Substances and Materials:   Complete if the study design includes administration of test materials or devices.  Common methods for administering materials include injection, vascular access, inhalation, surface application or surgical implantation.

Appendix 3-Specimen Collection: Complete if the study design includes the collection of blood or other body fluids, tissues, or devices.  This includes either pre- or post-mortem collections.

Appendix 4-Surgery:  Complete if the study design includes a surgical procedure.   This includes minor/major and acute/survival procedures in any vertebrate species.
