Parking is available in Lot E on Plain Street, close to the Physician's Office Building.

All RIte Care, Neighborhood Health Plan,United Health Plan, BC/BS, BlueChip, Tricare and other insurances accepted. Please call our office for further information.

Appointments and Other Information:

Please...arrive 15 minutes before your appointment time to register first.
Bring...immunization (shot) records, medication your child is taking, any inhalers and spacers, forms to be filled out, and any other medical records.

Appointments:

Physical Exams (including Camp and School Physicals)- Please schedule one month ahead by calling our office, Monday to Friday 9:00 am to 4:00 pm.

Sick Visits:

Please call ahead to be seen the same day or next day. A nurse or doctor will give you specific advice and an appointment will be made if necessary.

Appointment Cancellations:
If you cannot make your scheduled appointment, please call our office to cancel or reschedule at least 24-hours in advance.

Late Policy:
If you are more than 30 minutes late for your appointment we cannot guarantee that you will be seen. We will help you reschedule as necessary.

Forms:
School and camp physical forms, copies of immunization records,and WIC forms may need two days to complete.
